Advanced Registration Fees :
Full advanced registration for this conference is $425.00 dollars. The deadline for registration is April 13, 2001. The registration fee will include morning continental breakfasts, afternoon breaks, meeting space fees, registration night mixer, conference dinner on monday, workshop banquet at "Rawhide" (western style theme park) on tuesday night and two lunches. It also covers the proceedings booklet. After April 13, 2001 the late registration fee will be $525.00. No on-site registration will be offered. Please complete
the meeting registration form and mail with payment. If you have not received a registration
form in the mail, you can printout this
SIMS Workshop registration form, and mail or fax to the conference office.
Your registration must be accompanied by credit card information (VISA
or MasterCard Only) or a check in US dollars for the total amount. All
persons in the conference areas of the hotel must be registered and wearing
an official workshop name badge. Pre registered attendees may pickup their
name badge and registration packet beginning at 5 pm on Sunday, May 13, 2001
at the conference hotel.
Remember.... On site registration will not be offered.
To maintain the workshop nature of the meeting, the number of attendees will be limited to 150 people. Please register early.
Cancellation:
To cover the cost of handling and processing $30 will be deducted from refunds for cancellations. Written request for a refund must be postmarked on or before May 03, 2001.
